Broadband Connect Forms
Important notice
The Prime Minister and the Minister for Communications, Information Technology and the Arts announced the Australian Broadband Guarantee on 7 March 2007. From April 2007, the Australian Broadband Guarantee will replace the Broadband Connect Incentive program and the Metropolitan Broadband Connect program. All new provider registrations for the Broadband Connect program closed on 30 June 2006, no new provider registrations will be accepted after this time. Registered providers may submit variations till 31 December 2006.
All Broadband Connect Registration Forms received will be acknowledged. If you have not received an acknowledgment within 3 business days please contact DCITA at BroadbandConnect@dcita.gov.au.
Forms must be completed in full, and be accompanied by such supporting documents as specified in the Broadband Connect Guidelines and in the registration forms. Applications will not be processed until the fully completed application, including all attachments and supporting documentation, has been received by DCITA.
Broadband Connect provider registration form (File size: 381kb)
This is the application form for service providers. Applicants need to provide different levels of information according to their revenue and existing customer base. A completed Service Registration Form must be submitted with this document.
Broadband Connect wholesaler registration form (File size: 255Kb)
This is the application form for the registration of wholesale service providers. All applicants must provide the same level of information.
Broadband connect service registration form (File size: 344Kb)
This is the application form for the registration of Broadband Connect Services. Under the new Guidelines which take effect from 1 July 2006 and will therefore apply to all applications for registration under Broadband Connect prior to the cut-off of 30 June 2006 each applicant seeking registration must seek registration of at least one Broadband Connect Threshold Service and must seek registration of at least one Added Value Service. Applicants seeking to register more than one Broadband Connect Service Threshold and Added Value service should list all proposed services on the Multiple Services pricing table in addition to completing the Service Registration Form. A completed Provider Registration Form must be submitted with the Service Registration Form.
Multiple services pricing table (File format: XLS, File size: 19Kb)
Broadband Connect Providers seeking to register more than one Broadband Connect Service operating under the same technology platform/s, and where the service variables relate to speed, download and price should complete one Service Registration Form and list all services on the Multiple Services pricing table.
Broadband Connect new service area application form (File size: 185Kb)
This is the form Broadband Connect Providers will use when they wish to add a new coverage area for one or more of their Broadband Connect Services.
Financial plan template for Category C and D Broadband Connect applicants (File format: XLS, File size: 374Kb)
Applicants categorised as Category C and D Broadband Connect service provider applicants will need to provide a financial plan. This is the mandatory template for the plan.
