Metro Broadband Connect Forms
Important notice
The Prime Minister and the Minister for Communications, Information Technology and the Arts announced the Australian Broadband Guarantee on 7 March 2007. From April 2007, the Australian Broadband Guarantee will replace the Metropolitan Broadband Connect program and the Broadband Connect Incentive program. All Metro Broadband Connect Registration Forms received will be acknowledged. If you have not received an acknowledgment within three business days please contact DCITA at MBC@dcita.gov.au.
Forms must be completed in full, and be accompanied by such supporting documents as specified in the Metro Broadband Connect Guidelines and in the registration forms. Applications will not be processed until the fully completed application, including all attachments and supporting documentation, has been received by DCITA.
Metro Broadband Connect Service Provider Registration Form - (File format: RTF, File size: 856Kb)
This is the application form for service providers. Applicants need to provide different levels of information according to their revenue and existing customer base. A completed Service Registration Form must be submitted with this document.
Metro Broadband Connect Service Provider Streamlined Registration Form
Service providers already registered under the Broadband Connect program may be eligible to register through a streamlined application process at DCITA's discretion. Interested providers should contact DCITA to discuss this option. DCITA will forward the streamlined registration forms to approved service providers.
This is the application form for the registration of Infrastructure Providers. All applicants must provide the same level of information.
Metro Broadband Connect Service Infrastructure Provider Streamlined Registration Form
Infrastructure providers already registered under the Broadband Connect program may be eligible to register through a streamlined application process at DCITA's discretion. Interested providers should contact DCITA to discuss this option. DCITA will forward the streamlined registration forms to approved infrastructure providers.
Metro Broadband Connect Retail Service Registration Form - (File format: RTF, File size: 791Kb)
This is the application form for the registration of Metro Broadband Connect Retail Services. Each applicant seeking registration must seek registration of at least one Metro Broadband Connect Threshold Service and at least one Added Value Service providing a peak Data Speed of at least 512 Kb)ps to the customer and 128 Kb)ps from the customer as well as a minimum monthly usage allowance (download and upload) of at least 1 GB. Applicants may seek registration of additional Added Value Services.
Applicants seeking to register more than one Metro Broadband Connect Service Threshold and Added Value service are required to complete Part 2 for each of the additional services. A completed Provider Registration Form must be submitted with the Service Registration Form.
Metro Broadband Connect Wholesale Service Registration Form (File format: RTF, File size: 724Kb)
This is the application form for the registration of Metro Broadband Connect Wholesale Services.
Applicants categorised as Category C and D Metro Broadband Connect service provider applicants will need to provide a financial plan. This is the mandatory template for the plan.
Metro Broadband Connect Statutory Declaration (File format: RTF, File size: 36Kb)
Each application must be submitted under the cover of a Statutory Declaration. For example, the initial Service Provider and Service registration forms may be submitted under cover of a single Statutory Declaration. Subsequent Service registration forms must be submitted under cover of a new Statutory Declaration.
